The Diagnostic Accuracy of Purple Line in Prediction of Labor Progress in Omolbanin Hospital, Iran [1]
The Diagnostic Accuracy of Purple Line in Prediction of Labor Progress in Omolbanin Hospital, Iran. Currently, vaginal examination is the gold standard for assessment of labor progress
Therefore, this study aimed to determine the diagnostic accuracy of purple line in the prediction of labor progress.. In this cross-sectional study, 350 women with a single pregnancy in vertex presentation and gestational age of 38-42 weeks without any medical disorder, admitted to government hospitals of Mashhad, were selected using convenience sampling
Abnormal progress of labor was defined as cervical dilatation less than 1 centimeter/hour in the active phase for two consecutive hours and fetal head descend less than 1cm/h or duration of more than two hours for nulliparous and one hour for multiparous In the second stage of labor. Data was analyzed by SPSS version 16 using chi -square test.
There’s This Thing Called the “Purple Line” — Wichita Doula [2]
Before you have a baby, you never knew how much you never knew. I’m about to drop some more knowledge on you today.
This study of 144 women in labor found that 109 of the women had this purple butt line, and to some degree it did correlate to the percentage of cervical dilation. I’ve even personally observed it when attending births as a doula!
This line can be very faint, or as bold as if you drew it on with a felt-tip marker, and it grows vertically. Some say that when it reaches the top of your crack, that means you’re fully dilated.
1-centimeter dilated: What it means and when will labor start? [3]
Dilating to 1 centimeter does not necessarily mean that labor is only hours or days away. The cervix can be dilated to 1 centimeter for weeks before the beginning of labor
Most pregnant women spend some time wondering when they will go into labor, especially as the due date draws near. When the opening of the cervix starts to widen, this is called dilation, and it is one sign that labor is approaching.
In this article, we look at what dilation is and what dilating to 1 cm signals. We also describe other signs that labor may start soon.
The Purple Line of Labor progression: Myth or Magic? — 4th Trimester Fitness Method [4]
The Purple Line of Labor progression: Myth or Magic?. Did you know that the Purple Line is seen in about 76% of active laboring women and can be used for estimating progression of labor and station of baby? Really! How crazy is that! It is more readily seen in women who are of white ethnicity and who have experience a premature rupture of membranes (1)
But how in the world does that work? Well, as the baby descends into the deep pelvis, the purple line rises up from the outskirts of the anus. This appearance has been thought to occur because of the vascular congestion at the base of the sacrum
But fortunately, Mother Nature helps me out by giving a bunch of clues about progression of labor: vaginal discharge, contraction pattern and intensity, vocalization changes, pelvic floor pressure, changes in demeanor to shakes / chills, and…. So, just where is this purple line to be found? Back in my midwifery days, we called it the “Natal Cleft”, but most folks just call it the “butt crack”! Natal Cleft sounds more like something exciting is about to happen, doesn’t it? I kind of like that!
Find Out If You Are Dilating [5]
There is no doubt that the most reliable way to tell, if you are dilating and also how much you are dilated is a vaginal exam. However, a vaginal exam is neither a very pleasant experience nor always an option.
You just need to know where to look and how to interpret what you see!. One of the most remarkable ways in which your body MAY be able to tell you that you are dilating is the so-called Purple Line! It is a purple or brownish line (mostly depending on skin color) that extends from a woman’s anus upwards to the top of the cleft between her buttocks!
However, from our personal experience, we very much doubt that 76% of women develop a purple line.. We’d rather say that 76% of women do NOT develop a purple line
The purple line as a measure of labourprogress: a longitudinal study – BMC Pregnancy and Childbirth [6]
The purple line as a measure of labourprogress: a longitudinal study. BMC Pregnancy and Childbirth volume 10, Article number: 54 (2010)
It is however inherently imprecise with studies indicating an overall accuracy for determining the diameter of the cervix at between 48-56%. Furthermore, VEs can be unpleasant, intrusive and embarrassing for women, and are associated with the risk of introducing infection
The presence of a purple line during labour, seen to rise from the anal margin and extend between the buttocks as labour progresses has been reported. The study described in this paper aimed to assess in what percentage of women in labour a purple line was present, clear and measurable and to determine if any relationship existed between the length of the purple line and cervical dilatation and/or station of the fetal head.

Studies also show that vaginal exams are not really accurate. When checking for exact dilation, studies show the accuracy to be around 48-56%
And another study abstract which was done on models.] When you add in multiple people checking, the accuracy gets even worse. Yet, vaginal exams are considered the “gold standard” of assessing labor progress

Cervical Dilation with Purple Line [9]
Has anyone heard of this? It’s a science backed study that showed a correlation between a red/purple line on the butt crack of the expectant mother and how dilated her cervix was. It occurs in about 75% of woman with a 50% accuracy
If mine is correct, it is about 6.5-7cm long meaning I’m about 2.5 cm dilated. My appt is tomorrow so I’ll see if there’s a connection
– I heard of this, but the few people I know who checked it themselves were wildly off! I think some midwives may use it? They’d probably be more accurate than a self inspection. After seeing a couple posts I finally looked lol I definitely have the line but idk how dialated I am because we haven’t checked in a couple weeks and won’t again until I’m in labor
What is the Purple Line in Labor? — GentleBirth [10]
Purple Line Dilation – What is the Purple Line in Labor?. More parents are opting OUT of uncomfortable routine vaginal examinations in labor and prenatally (without a medical indication) and are opting IN to the purple line dilation club as a way of assessing labor progress
A brand new 2021 study suggests it is about 85% accurate for dilation and how far down into the pelvis baby is. Yes dilation is important – everyone is obsessed with that number but the station of your baby (how high/low in the pelvis) is crucial information too
Ideally being at +2 ‘station’ is when you’ll start to feel that strong urge to push. If you have an epidural allowing your body to nudge baby down takes a LOT of the ‘work’ out of pushing.
Evidence for the purple line… [11]
Can you help me find references / evidence relating to the purple line?. There aren’t many at all, but there are a couple of important articles and a research study for those wanting to read more and/or write about this sign of labour progress.
In early labour, the line is short, and it gets longer as labour progresses.. Some midwives are good at telling how far along a woman is in her labour by looking at the purple line.
Which means that it is not quite so useful when women choose positions other than this and/or if they don’t want to have someone staring at their bottom – which I don’t think is at all unreasonable!. The purple line was first discussed in the literature (to my knowledge, but please see below) in a letter written to The Lancet by Byrne and Edmonds (1990).
Purple Line Dilation [12]
When you’re in or approaching labor, you may develop a purple line that runs vertically from your anus to the top of your natal cleft in between your butt cheeks. This “purple butt line,” as it’s often called, is said to first appear when the cervix is between zero and two centimeters dilated
This purple line may be the answer to one of the more difficult roadblocks people often face during labor: How to check for dilation in a way that’s comfortable.. Please Note – This guide includes an extensive selection of purple line dilation pictures from real world birth journeys.
The purple line theory is a simple method for self checking dilation that could be used by birthing people and their supporting people that is less intrusive than traditional cervical exams. The theory is based on observed data from midwives and physicians
Vaginal examinations aren’t the only way to tell how your labour is progressing [13]
Vaginal examinations aren’t the only way to tell how your labour is progressing. Did you know that there is a way to tell how far a woman’s labour is progressing that doesn’t require a vaginal examination? If you didn’t – this fun fact might just blow your mind.
It begins at the anus, and as her labour progresses, it moves all the way up to the top – indicating full dilation. Being informed about this line and watching its movement can equip a woman and her care providers with the knowledge of how things are going without having an intrusive vaginal examination.
Senior Lecturer in Midwifery, Dr Rachel Reed concurs. She says that knowledge of the purple line is one assessment she uses to ascertain how labour is going for the women under her care, and emphasises the importance of observing the whole woman – not just her cervix.
What is the purple line in pregnancy? [14]
The “purple line” in pregnancy is a vertical line of discoloration seen on a pregnant woman’s abdomen which appears during the third trimester. This line, which runs from the belly button to the pubic bone, is caused by the expansion of the rectus muscles due to the growing uterus.
The medical term for this phenomenon is linea nigra and it is a completely normal and common part of a pregnancy.. No, purple line does not necessarily mean that Labour is near
However, the purple line does not necessarily indicate the closeness or success of any particular party.. It is often used in opinion polls and surveys to indicate a hypothetical or actual majority party, but it does not necessarily mean that party is close to gaining a majority or winning the election
For many women, ‘the purple line’ appears during childbirth. This is how midwives use it. [15]
Traditionally, the birthing process was kept behind a veil of ‘secret women’s business’ type mystery and privacy.. Thankfully, in the 21st century we realise that this approach has been helpful to precisely no one.
Watch: Questions about childbirth answered by mums and non-mums. It is therefore pretty surprising to read something that relates to childbirth that even after having two kids of my own, I knew nothing about.
This line doesn’t just show up for fun like a stretch mark – oh no. It can actually be used by midwives as an indicator for how your labour is progressing.

Dive into anything [17]
Hi! I’m 38+3 right now and I have heard this talk about measuring dilation by the purple line that creeps up your butt crack (pardon my lack of medical politeness) and I’m curious whether anyone has had their dilation or labor progress measured using this method, and if it was accurate, as based on where mine is, I should practically be ready to push but have had no signs of active labor at all. I’d love to imagine I’m one of the 1% of people that don’t experience pain during labor but that seems like too much of a stretch for me just considering my pain threshold in general.
She said she hadn’t heard of it but would check it out after the appt.. ALSO, the cervical check that I got measured me at 2cm dilated and 50% effaced, with baby’s head engaged at a 0
So NOT an accurate dilation indicator based on my personal experience, but who knows.
